The goods and services shown below are those we can provide to our customers. You may choose only the items you desire. However, any funeral arrangements you select will include a charge for our basic services and overhead. If legal or other requirements mean you must buy any items you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the funeral goods and services you selected.
STATEMENT OF DISCLOSURE OF PRENEED
“Prior to drafting any contract for goods or services, the responsible party or the decedent’s survivor who is handling the funeral arrangements is entitled to receive a copy of any preneed agreement in the possession of the funeral establishment that has been signed and paid for in full or in part, by or on behalf of the decedent”.
TRADITIONAL SERVICE SELECTIONS
Local Church or Chapel Service
Includes: Basic services of funeral director and staff, embalming, dressing, casketing, and cosmetology, use of facilities and staff for two hour viewing/visitation, use of facilities and staff for funeral service in our chapel or local church, or equipment and staff services for funeral service in another facility, transfer of deceased to funeral home, funeral coach and utility vehicle.
Graveside Service
Includes: Basic services of funeral director and staff, embalming, dressing, casketing and cosmetology, use of facilities and staff for two hour viewing/visitation, used of equipment and staff for local graveside service, transfer for deceased to funeral home and use of funeral coach.

ITEMIZED SERVICE SELECTIONS
Basic Services of the Funeral Director and Staff
Our service includes, but is not limited to, attendant on duty 24 hours a day to respond to the initial request for service, care and sheltering of the deceased, preparation and filing of the death certificate and disposition permit, consultation with the family and coordinating service plans with cemetery, crematory and others responsible for final disposition of the deceased. This fee also includes a proportionate share of our basic overhead costs such a facility maintenance, administration, insurance, etc.
This fee for our basic services and overhead will be added to the total cost of funeral arrangements you select. (This fee is already included in our charges for Direct Cremations, Immediate Burials, and Forwarding and Receiving Remains).
Embalming
Except in certain special cases, embalming is not required by law. Embalming may be necessary, however, if you select certain funeral arrangements, such as a funeral with viewing. If you do not want embalming you have the right to choose an arrangement that does not require you to pay for it such as direct cremation or immediate burial.
Other Care of the Deceased
Includes the following services as needed:

Special Care of Autopsied Remains
This service includes the extra time and materials needed for restoration after an autopsy and is in addition to our embalming charge.
Special Care of Tissue or Transplant Donors
This charge is for special restoration and is in addition to our embalming charge.
Sanitary Care of Unembalmed Remains
Refrigeration
It is the policy of our firm to require refrigeration of an unembalmed deceased after 24 hours of death.
FACILITIES AND EQUIPMENT
Use of Facilities and Staff for Viewing/Visitation (2 hours)
Our service include setup and use of visitation room, placement of floral arrangements and staff in attendance for visitation.
Use of Facilities and Staff for Funeral Service in our Chapel or
Equipment and Staff Services for Funeral Services in another
local Venue
Our service includes coordinating the funeral arrangements, supervision of funeral service and staff participation in funeral service in our chapel or in another local venue.
Use of Equipment and Staff for Local Graveside Service
Our services include accompaniment of the deceased to cemetery, supervision of graveside service and staff participation.
Use of Facilities and Staff for Funeral Service in our Chapel or Equipment and Staff for Funeral Service in another local Facility followed by Cremation.
Our services include coordinating the funeral arrangements, supervision of funeral service and staff participation.
Use of Facilities and Staff for Memorial Service in our Chapel
or Equipment and Staff for Memorial Service in another
local Facility (without deceased present)
Our services include coordinating the Memorial Service arrangements and staff supervision.
Use of Facilities and Staff for Rosary or Vigil Service in our Chapel or Equipment and Staff for Rosary or Vigil Service in another local Venue.
Our services include supervision of prayer and vigil service and staff
assistance.
Private Viewing (Family only, up to 1 hour)
Overtime for exceeding the normal two hour service time.
Use of Facilities, Equipment and Staff on Saturday, Sunday
or Legal Holiday
Direct Cremation (without ceremony)
Our charge for a Direct Cremation without any attendants, rites or
ceremonies includes:
Basic services of funeral director and staff
Transfer from place of death within 25 miles of the funeral home
Shelter/Refrigeration of remains
Transportation to crematory
Crematory fee
Direct cremation with container provided by purchaser
Direct cremation with container purchased at the funeral home
Immediate Burial (No viewing or ceremony)
Our charge for an Immediate Burial includes:
Basic services of funeral director and staff
Transfer of the deceased within a 25 mile of the funeral home
Shelter/Refrigeration of remains
Funeral coach to local cemetery
Immediate Burial with casket selected at funeral home
Immediate Burial with casket provided by purchaser
Forwarding Remains to another Funeral Home
Our charge for Forwarding of Remains to Another Funeral Home includes:
Basic services of funeral director and staff
Transfer of the deceased from place of death within a 25 mile radius
Embalming and other preparation of the body
Service utility vehicle.
A casket and shipping container must be purchased in addition.
Beyond a 25 mile radius mileage extra charges apply per mile.
(Visitation, rites, ceremonies or shipping costs are not included)
Receiving Remains from another Funeral Home
Our charge for Receiving Remains from another Funeral Home includes:
Basic services of funeral director and staff
Transportation within 25 miles of the funeral home.
Funeral coach to the cemetery.
(Visitation rites and ceremonies are not included)
MERCHANDISE
All Merchandise is subject to applicable sales tax
Caskets, Urns, Alternative containers and Shipping containers
A complete list will be provided at the funeral home.

Includes a of standard memorial book, (100) standard memorial folders and acknowledgement cards & envelopes.
